The student named in this application has the permission of the undersigned to participate at Organized Kaos (OK). Students will be required to participate in safety training or they will be unable to participate. Students will be operating handheld and stationary power equipment. Each tool must be taught and the student must demonstrate safe usage in order to use. Safety equipment must be worn when using equipment. Mentoring will be done in ratios of no more than one trades person to five students.

In the event of an emergency affecting the health or welfare of this participant, the staff or volunteers have permission to administer first aid and/or transport the individual to the nearest doctor or hospital for further medical attention, as deemed necessary. The individual action in response to the emergency will be held blameless. Any medical expenses occurring will be borne by the parents or guardians of the participant. The undersigned agrees not to hold Organized Kaos or its employees/volunteers liable for injury, loss, or damage to person or property resulting from participating in these events.
